
This website provides resources, creates worker networks and gives suggestions to anyone involved with young people, particularly professional case workers. It explains about our Intensive Case Management service, which operates in the North and West Metropolitan region for case workers working with young people who are experiencing homelessness.
The focus of the service is on young people with high and complex needs, when the case worker feels that engagement, stabilization, progress and/or outcomes for the young person are not being achieved as planned.
The service aims to provide support and resources to assist workers to maintain and enhance engagement, to consider different strategies to encourage stability and progress and to consider case planning goals and issues to assist positive changes occurring for the young person and case worker.
The website also provides links to other services and websites. The resource list is not exhaustive and will continually be updated to include appropriate and youth friendly services.
